What a good tenant maintenance request form includes

Most landlord maintenance problems aren't maintenance problems — they're intake problems. A tenant texts vaguely about a leaking faucet, you respond eventually, you forget what they said, you call them back the next day and they're at work, and suddenly the same leak has become a ceiling stain.

A standardized request form solves the first 70% of that. At minimum, every form should capture:

  • Who: tenant name, unit/property, date
  • What: issue type + written description, ideally with photos
  • How urgent: a clear 3- or 4-step urgency scale with definitions
  • When they're reachable: time windows when you or a contractor can access the unit
  • Preferred contact method: text, email, or call
  • Signature + date: dated paper trail

When to use a paper form vs a digital tool

Paper forms are unbeatable for three situations:

  1. Establishing the process with new tenants.Printing the form, handing it at move-in, and explaining “fill this out when something breaks” is a clear ritual. Way more effective than a quick text with a link.
  2. Legal paper trail. Dated + signed forms are defensible in security-deposit disputes, small-claims court, and landlord-tenant hearings. Screenshots of texts are not.
  3. Older tenants or low-tech households.Some tenants genuinely prefer handing you a filled-in piece of paper. Don't fight it.

Digital tools win when you scale past 2–3 units. The paperwork gets unwieldy, you lose track of who submitted what, and the status-update loop (“I'll let you know when the plumber comes”) eats your evenings. Three common mistakes on maintenance forms

1. No urgency definition

“High priority” means different things to a tenant vs a landlord. If you don't define urgency levels, everything becomes “high” and you lose the signal. Our template includes plain-English definitions: low (aesthetic), normal (functional but not urgent), high (impacting daily use), emergency (call, don't fill out a form).

2. No photo guidance

Photos matter more than descriptions. A 2-sentence description of “kitchen sink leaking” covers ten different jobs ranging from a $15 washer to a $400 disposal replacement. A photo narrows it immediately. The form should ask for photos explicitly — “Photos attached: yes / no” — so tenants don't forget.

3. Mixing emergencies into the form

Tenants will fill out a form at 2 AM about a burst pipe if you let them. Put the emergency phone number abovethe form fields, in bold, with a line that reads: “For emergencies (fire, flood, gas, no heat in winter, no hot water 24h+) call ___ instead.” Our template has this built in.

How to roll this out

  1. Fill in the property + contact fields once, then save the PDF.
  2. Print 10 copies per unit.Staple one to the inside of the lease binder, hand the rest to the tenant at move-in. Tell them: “When something breaks, fill this out, text me a photo of it, I'll handle it within 24h.”
  3. Keep filled-in copies. Start a folder per unit. When the tenant moves out, every request they ever submitted is right there.

FAQ

Can I customize the form further than your template allows?

Yes — the PDF is unlocked. Open it in Adobe Reader or Preview and annotate, add your logo, cross out fields you don't want. If you need something substantially different, email us the changes you want at hello@fixqueue.appand we'll probably add it to the template.

Is there a Spanish version?

Not yet. If you'd use it, email us — we'll prioritize based on demand.

What about a fillable PDF I can email?

A fillable form field version is on the roadmap. For now, tenants can open the PDF in Preview or Adobe Reader and use the “Fill & Sign” tool to type directly.
